IMPORTANT
PRINTING INSTRUCTIONS
Some
of the forms are two pages. They have been set up to print on two
sides of one paper.
Print PAGE ONE only. Then turn the paper over and print page two.
The margins have been set up to accommodate holes in the left margin
so that the forms can be stored in the binder.
